At Speonk Physical Therapy we pride ourselves on providing a unique treatment plan for each individual patient. For your convienience, we have provided links to our office forms that must be filled out before your initial evaluation.
The Patient Health Questionnaire must be filled out by each patient. In addition, please select the functional form that best fits the injury/bodypart causing your problem.
If you would rather fill out these forms in the office please be sure to arrive 10 minutes early to allow ample time for paperwork before your evaluation.
FAQ
What does the Inital Evaluation Entail?
Your first visit will include a comprehensive initial evaluation performed by a licensed and insured physical therapist. Depending on your specific case, we may begin treatment the first day as well. Through differential diagnosis, Kelly evaluates her patient’s injuries to determine the underlying cause of the patient’s symptoms. Based upon evaluation, a custom plan of care is devised to assist with regaining pain free independence with all activities of daily living. During your first visit your physical therapist will discuss the following with you:

Your complete medical history.

Your current problems/complaints.

Pain intensity, what aggravates and eases the problem.

How this is impacting your daily activities or your functional limitations.

Your goals with physical therapy.

Medications, tests, and procedures related to your health.

A detailed treatment plan that uniquly fits your needs

In most cases, a home-exercise program.

You will be given a print out of your home-exercises by your therapist.

What Do I need to Bring to My First Visit?

Your prescription for physical therapy.

Your insurance card
-
A valid photo identification.

A list of all Current medications including frequency and dosage.

Reading glasses to fill out paperwork.

You can download the patient forms from our website (see above). If you wish to complete the forms in the office please arrive 10 minutes early.

How long will each visit last?
Physical Therapy sessions generally last 60 minutes.
What should I wear?
It is best to wear loose fitting clothing to your appointments. It is helpful if we can see and palpate the area of injury/pain. For example, if you are coming to PT for a knee problem it is best to wear shorts or loose pants that you can pull up above your knee.
